It's a film that came out a lot earlier. It's not one I've seen though. I remember seeing it (and others I can't identify) at the video store close to 10 years ago.

Sorta looks like Dog Soldiers (but too new). Sorta looks like Michael Jackson in Thriller.
But I'm pretty sure it's The Howling (link to model of werewolf). Screencap of the movie here. There are plenty of "I'm turning into a wolf" pics, but I can't seem to find a decent screencap of the fully evolved werewolf

#4 has the hat like the Creeper, and the ax. Creeper's face.
But the face looks too human. (That being said, I'm 90% sure it is the Creeper).
#5 I agree is the Urak-Hai. Though I keep thinking there's something else. Something that gives me a Pumpkinhead vibe. By vibe, I mean I have the impression that I've seen that monster on a farm, in a scene similar to where I've seen Pumpkinhead.
#15 is from The Howling. Werewolves look different.
Where can we get answers? I think I had a few more of those right. I'll be amazed if I called "The Reanimator" correctly.

I found too many disturbing things to continue my search for answers. The graphic depictions of werewolf rape shall haunt my nightmares for weeks to come.

The screen shot you posted, Paul, is from later on in the flick when he's more monster-looking. Earlier, he does sport the trench coat & hat & looks considerably more human.
As for #5, the only reason I say the tamed zombie from Day of the Dead was that a scientist managed to domesticate him by the military dude in charge of the based still insisted he be chained up. & as far as zombies go, he's one of the more memorable & important ones.
