2. He's in prison for life.
3. Nobody needs a sex change to live, especially when they've got no place to fucking go until they die.

There are laws and regulations which identify gender reassignment surgery as "non-cosmetic." The real bit was trying to prove how this was the "least intrusive, minimal means" to resolve the situation.
I find it difficult to be angry at people who are exploiting a broken system, at least more so than the fuckwads who broke it in the first place.
3. Nobody needs a sex change to live, especially when they've got no place to fucking go until they die.
The law and various Supreme Court rulings contradict you.
